Woman spends £180,000 on Scotland’s largest boobs – now she needs them even greater
A glamour model who says she has Scotland’s biggest breasts has spent £180,000 on her “living doll” transformation and now plans to go bigger again to 3,000cc despite past implant complications
A woman who claims to have Scotland’s biggest breasts has spent £180,000 transforming her body, and says her famous chest has almost taken control of her life.
Ayla Smith, 31, from the Isle of Skye, currently has enormous 2,800cc breast implants after undergoing two further enlargements since February 2025. The café owner and professional glamour model is now planning to increase them to 3,000cc during another operation in December.
But before then Ayla is booked to spend a further £10,000 on a full-face transformation in September, including a “Barbie nose” and facelift. The upcoming procedure will take her surgery and aesthetics spending to at least £190,000.
Ayla, who has owned The Silver Seal Café for eight years, began transitioning when she was 24. By February 2025, she had undergone 15 operations, including forehead reconstruction, an eye bone shave, a nose job, lip lift, two jaw shaves, chin surgery, an Adam’s apple shave, veneers, six breast operations and gender reassignment surgery.
Since then, she has had her jaw shaved down again and undergone two further breast enlargements, taking the number of procedures to at least 18. However, the pursuit of her extreme ‘living doll’ look has come with frightening consequences.
During a previous breast enlargement, part of Ayla’s incision failed to heal and one of her implants became exposed. It eventually had to be removed, leaving her with one dramatically enlarged breast and one completely flat side.
Ayla says: “When I lost my implant, it was the worst time of my life. I felt hideous having one large breast and one completely flat, but that’s life.
“I accepted it, I got through it and I’m now bigger than I was then. I’m always a bit scared, but my boobs are famous.
“They’re kind of above me when it comes to making these decisions.” Despite the devastating complication, Ayla remains determined to reach her 3,000cc target.
She admits that doctors have raised concerns about the safety of her enormous implants and the risks involved in making them even bigger. Ayla says: “With my boobs, the doctors obviously don’t think it’s safe, and of course it’s risky.
“With my face, surgeons in the UK are more conservative, and safe. They err on the side of caution and don’t push things too far.”
Her oversized implants also affect her daily life. Ayla finds sleeping on her side uncomfortable, even though it is her favourite sleeping position.
She can no longer run as she once did and must be careful around weights and certain physical activities. She says: “My implants make it a little uncomfortable to sleep on my side, which is hard because that’s my favourite sleeping position.
“I definitely can’t run like I used to. I also have to be careful and mindful around weights, not that I lift them.
“But it’s all part of the life. If anything, they help me in the café.”
Ayla balances running The Silver Seal Café with her work as a professional glamour model. Her husband, David, supports her transformation and loves her extreme appearance.
Ayla says: “David loves my look. He is just worried about me looking like everyone else.”
Ayla does not have children of her own, although David has two children from a previous relationship. The couple hope to have a baby together within the next few years.
