Plastic surgeon points dire warning over ‘most threatening’ celeb-loved procedures

A plastic surgeon has revealed the cosmetic procedure that he would never do himself – and it’s one that has taken over Hollywood in recent years.

The admission was made by Oregon-based plastic surgeon Dr Sean McNally, who is the lead provider at Doctor PDX and Réviance in Portland.

Dr McNally has expertise in both cosmetic and reconstructive surgeries, specializing in facelifts, tummy tucks, breast procedures and hand procedures. 

However, there are two things that he would never do himself – the first one being butt implants.

‘[They have] very high long term complication rate regardless of how it’s done,’ Dr McNally told the Daily Mail exclusively of the procedure.

‘That’s why we shifted to BBLs,’ he continued.

And, on a similar note, Dr McNally said that he would never get fat grafting into the gluteal muscles during a Brazilian Butt Lift, more commonly known as a BBL.

It’s a procedure that’s been a hit in Tinsel Town, as rapper Cardi B has gone under the knife for one, as well as Nicki Minaj.

Buttock augmentations surged by nearly 63 percent worldwide between 2020 and 2024, with plastic surgeons performing over 644,000 BBL procedures this year alone, according to international data. 

Typically, BBLs involve taking fat from other parts of the body and injecting them into the thighs and butt areas. 

However, injecting the fat into the muscle has proven to be dangerous, according to Dr McNally. 

‘Initially this was started to give more space to put fat for BBL but turns out it increases the rate of fat embolism which made BBL the most deadly plastic surgery,’ Dr McNally explained to the Daily Mail of the fat grafting element of the procedure.

‘Now we’re told to avoid injection into the muscle and to use ultrasound to make sure we’re in the right space for fat injection,’ he said.

And, according to the American Society of Plastic Surgeons, further complications from inadvertently injecting fat intramuscularly can also include fat necrosis.

Fat necrosis is a ‘death of fat tissue,’ according to Cleveland Clinic, and can appear like lumps and bumps under your skin.

There can be multiple complications stemming from fat necrosis, like serious infection.

In fact, it’s a surgery that people often regret the most.

In November, Dr Fuat Yüksel, a plastic surgeon at the UK-based firm Longevita, told the Daily Mail that the the primary complaint from his patients is: ‘The result did not meet my expectations.’

According to Dr Yüksel, many patients enter the operating room with unrealistic expectations. In reality, there is a limit to safe fat transfer and a significant portion of it will be naturally reabsorbed by the body as it heals.

Grammy-winner Sza told British Vogue last year: ‘I’m so mad I did that sh**. I gained all this weight from being immobile while recovering and trying to preserve the fat. It was just so stupid.’
